I.     ACCEPTABLE BIRTH DOCUMENTS

*The birth date must be confirmed by at least one (1) of the following Documents:

1.     Certified copies of birth certificate, which indicates a true and correct copy

2.     Original baptismal paper with seal made within one year of birth.

3.     Immigration papers – original

4.     Hospital certificate – original

5.     Passport

6.     Birth certificate copy indicating as “true and correct” and notarized by a notary public.

7.     Current valid Louisiana Driver’s License

 

II.               ACCEPTABLE RESIDENCE VERIFICATION DOCUMENTS

*The address must be confirmed by at least one (1) of the following Documents:

1.     Parents’ current driver’s license.

2.     Any home utility bill, to include cable television.

 

III.           ACCEPTABLE COLORS:

          Only Black and Blue cards are valid.

 

IV.           COSTS

A.   $1.00 for regular ID sessions.
